Upon notification of an identity theft incident, Privacy Advocates will act on your behalf as a dedicated case manager.
Privacy Advocates will assist members in the event their credit cards are misplaced or stolen.
You authorized Concierge Benefit Services to charge the credit card or ACH debit indicated in this authorization. If the above noted payment dates fall on a weekend or holiday, you understand that the payments may be executed on the prior business day. You understand that this authorization will remain in effect until you cancel it in writing, and you agree to notify Concierge Benefit Services in writing of any changes in your account information or termination of this authorization at least 15 days prior to the next billing date.
